Statement of Information Practices
At Lakeland Therapy Centre, we understand that privacy and confidentiality are essential to a trusting therapeutic relationship. We are committed to protecting your personal health information and maintaining your privacy in accordance with Ontario’s Personal Health Information Protection Act (PHIPA).
Collection, Use and Disclosure of Personal Health Information
As a psychotherapy clinic, Lakeland Therapy Centre collects personal health information that is necessary to provide and manage your care. This may include information about your health history, treatment, contact information, clinical notes, and other information relevant to your care.
We use your personal health information to provide and coordinate your therapy, communicate with you regarding your care and appointments, maintain your clinical records, process payments and administrative matters, and meet our legal, professional, and regulatory obligations.
Personal health information is only disclosed with your consent or when permitted or required by law. There are certain circumstances under PHIPA and other legislation where information may be disclosed without consent. Your therapist will discuss the limits of confidentiality with you as part of your care.
Lakeland Therapy Centre also uses electronic systems to maintain and manage client records. We take reasonable steps to protect personal health information against unauthorized access, use, disclosure, loss, or theft.
Confidentiality
Confidentiality is one of the core elements of a trusting therapeutic relationship and hence can not be downplayed. Please be aware that I keep your confidentiality in highest regard.
By law (the Personal Health Information Protection Act or PHIPA) , a therapist is to keep a client’s personal health information private, unless consent to release the information is provided by the client.
I am also required to keep your records secure.
Under some circumstances a therapist is required to divulge some aspects of this information. I routinely discuss any limits of confidentiality with my clients and can speak with you about any inquiries you may have.

Video Surveillance
Lakeland Therapy Centre uses outdoor video surveillance for the safety and security of our clients, staff, and property. Surveillance is limited to exterior areas of the clinic.
Your Access to Your Information
You have the right, subject to certain legal exceptions, to request access to your personal health information held by Lakeland Therapy Centre.
You may also request a correction to your personal health information if you believe that a record is inaccurate or incomplete.

Requests for access to or correction of records should be made in writing and provide sufficient information for us to identify the records being requested.

If you have a question or concern about how your personal health information is collected, used, disclosed, or protected, we encourage you to contact us first so that we can address your concern.
If you are not satisfied with our response, you may make a complaint to the Information and Privacy Commissioner of Ontario (IPC).
